Draw up a help plan

    Regardless of which form of "help for upbringing" you apply for at the Youth Welfare Office, the Youth Welfare Office will draw up a help plan with you, possibly your child and others involved. In the help plan discussion, it is worked out which help is appropriate for the problem at hand and which measures should be taken.

    In the help plan, the cornerstones of the help are laid down, such as:

    • Demand
      • Facts from your family (educational situation, economic circumstances, parents' behaviour, care of the child, physical and mental condition and the like).
      • if you have already made use of other assistance measures: What were they? Why was the help not successful?
      • Personal data sheet of your child (e.g. who are the parents, siblings, relatives, abnormalities, illnesses, allergies, vaccinations)
    • Aim of the aid measures
      • What is supposed to change as a result of the aid?
      • What are the expectations of the stakeholders?
    • Type of aid
      • Which services are considered necessary and suitable (e.g. full-time care, parenting support, socio-educational family support)?
      • Suggestions for action for parents, professionals, carers
      • Time horizon (How long should the assistance last?)
      • Necessary benefits (e.g. care allowance, initial equipment, therapy costs, travel costs, financial support )

    Note: The youth welfare office must inform all parties involved about the possible consequences of the planned help for the child or young person. It must also document this in the assistance plan:

    • the way in which the stakeholders work together; and
    • the extent of the counselling provided by the caregivers

    All those involved in the help plan sign the help plan.

  • Process flow

    In addition to the people directly involved (parents, children), the following people, for example, can also participate in the preparation of the help plan:

    • Related,
    • Doctors,
    • The child's school
    • if necessary, a psychological service

    Together, all those involved agree on the key points of the planned help. They determine regular further meetings of the participants (help plan discussions). These meetings serve to review the progress and success of the services. The assistance plan is updated, i.e. any necessary or effected changes to the assistance and measures are recorded.

    Note: In the assistance plan, the approved assistance is fixed for a certain duration. Do you need help for a longer period of time or can the help be terminated earlier? Then an extension or shortening is possible in consultation with the Youth Welfare Office.

    The guardians and the youth welfare office sign the assistance plan. If full-time care is planned as a support measure, the foster parents must also sign. All parties involved receive a copy.
