Regional Transition Management

The Waldeck-Frankenberg district supports young people aged between 12 and 26 inclusive during their transition from school to work, with the aim of enabling them to participate in society on an equal footing. Social education specialists, known as ‘transition guides’, offer individual support tailored to the needs of each young person. They help them to successfully overcome challenges at school and to develop and realise their career prospects. Support in coping with psychological or social pressures may also form the focus of the support provided.

The aim is to provide effective support to young people with special needs as they make the transition from school to working life, thereby enabling them to lead independent lives in the long term.

As part of this task, coordinated cooperation between the relevant authorities across different legal systems may be required in order to provide tailored and effective support. Regional transition management plays a key role in this regard, acting as a facilitator and guide.

The regional transition support service has several contact points in the participating general education and vocational schools and, together with its outreach work, covers the entire rural district.

 

  • Regional Transition Management offers:

    • Solution-focused one-to-one counselling for pupils, both in and outside school, as they make the transition from school to work
    • Career guidance (including a mentoring role)
    • Support with finding an apprenticeship and preparing application documents
    • Help with problems relating to your school-leaving certificate
    • Small-group work on specific topics
    • Support for migrant pupils experiencing integration difficulties
    • Support with applying for and arranging private tuition
    • Support for tackling active and passive truancy
    • Collaboration with school social workers, teachers and careers advisers
    • Home visits to discuss matters with parents
    • Help with dealing with the authorities
    • Support even after leaving school
