Care centre and community care are reorganising
There has been a change in staff within the teams at the care centre and the community care service. Community nurse Sina Zembellini has recently started supporting her colleagues at the care centre in her role as a registered nurse. This meant it was necessary to recruit another community nurse for their dedicated team. The areas covered by the six community nurses now in post also had to be reallocated. As the new colleague, Tabea Zembellini is now responsible for Frankenau, Gemünden (Wohra), Haina (Kloster) and Rosenthal.
The ‘Community Care’ project in the Waldeck-Frankenberg district is funded by the State of Hesse. Community care workers are qualified nursing professionals; they provide impartial and free advice in people’s own homes, act as guides and help people find their way through everyday life. To this end, they are in constant contact with service providers in the region, such as care and support services, meals on wheels, home emergency call schemes and care homes. Once an individual’s support needs have been assessed, they can be referred to appropriate services, such as home care services or the provision of assistive devices.
According to SGB XI, strengthening preventive care to maintain independence will also form part of the community care service’s remit in future. Consultations mainly take the form of home visits, but can also be carried out at regional offices if required. A leaflet detailing the service areas and the telephone numbers of the community care workers is also available at relevant locations (local authorities, care facilities, etc.) and can be requested from the Care Support Centre.
